Overview

Background:

The Kodye Fassbender Memorial Scholarship was established to honor both Kodye's life and his struggle with mental illness. When Kodye died by suicide in May 2017, instead of shying away from a difficult topic, his family chose to start a conversation around mental health and suicide awareness. The world needs others to join this very important conversation. Kodye was a sophomore at Appleton North High School where he is fondly remembered for his bright smile and willingness to always help a friend or fellow classmate. Kodye had a quick-witted sense of humor, enjoyed the outdoors and was a proud American who supported both Wounded Warriors and Mission 22 organizations. The goal of this scholarship is to provide financial support to an Appleton high school senior who is pursuing a career in a mental health related field. It's time to end the stigma around mental health

Eligibility

Eligibility: (taken into consideration to determine recipients) • Graduating seniors from the following high schools: o Appleton East High School o Appleton North High School o Appleton West High School • Planning to attend an accredited educational institution as a full-time student, and planning to pursue a career in the mental health field, including: o Counseling – mental health counselor, addiction counselor o Psychology – clinical psychologist, neuropsychologist o Psychiatry – Mental health psychiatrist, psychiatric technician o Social work – mental health social worker, social & community service manager, victim advocate • Demonstrated leadership qualities and extracurricular activities • Preference will be given to applicants from Appleton North High School

Award Amount:

One non-renewable $1,000 scholarship award for tuition expenses.

Selection:

Recipients are recommended each spring by the Community Foundation Scholarship Selection Committee along with a family member.

Collecting the Award & Payment Procedure:

Recipients must fill out and submit Scholarship Acceptance Form and Scholarship Verification Form through the Scholarship Portal (https://www.grantinterface.com/Home/Logon?urlkey=cffvrscholar). Recipients are required to provide supporting documents as instructed in the Scholarship Verification Form, such as class registration/list with credits listed, proof of major, grade transcript, etc. After approval of online Scholarship Verification Form, the scholarship check will be paid directly to the school the recipient will be attending. The Community Foundation communicates with students via email; it is the student’s responsibility to keep their Scholarship Portal account updated with their current email address.

Loss of Eligibility:

Failure to register as a full-time student at an accredited educational institution, pursuing a career in a mental health field.
