Overview

Background:

Jacqueline Diane “Jackie” Johnson passed away on Saturday, October 21, 2017, in her sleep at the age of 52. Jackie was a dedicated employee of Fox Communities Credit Union for 33 years, rising to Assistant Vice President of Information Systems. In her role she served many staff members as a mentor and was very respected by all. Despite her busy career, she and her husband Tracy successfully raised their sons Nick and Jacob. Jackie was also a treasurer for her sons’ Boy Scout Troup for years and attended UW Fox Valley and attained an Associate Degree of Arts and Science. As her sons grew up, Jackie wanted to give back to her community, and she volunteered at most of the events that the credit union sponsored. The community started to recognize her as the face of Fox Communities Credit Union, with a smile at many events. She was very proud of the fact that she was returning to school after a long hiatus and thought it was funny that her and their boys were all going to school at the same time! Even through the adversity of all the many things going on in her life, she still found the time to make her education a priority. In 2018, this scholarship was established by Fox Communities Credit Union and her friends in loving memory of Jackie.

Eligibility

Eligibility: (taken into consideration to determine recipients)

• New or continuing students who will be attending the UWO-Fox Cities campus as a part-time (minimum 6 credits) or full-time (minimum 12 credits) student • Pursuing a degree in Information Technology or Computer Science • Minimum cumulative 3.0 GPA • Demonstrates community involvement

Award Amount:

One $1,500 non-renewable scholarship award, payable for the fall semester.

Selection:

Recipients are recommended each spring by the UWO-Fox Cities campus scholarship selection committee.

Collecting the Award & Payment Procedure:

Recipients must fill out and submit Scholarship Acceptance Form and Scholarship Verification Form through the Scholarship Portal (https://www.grantinterface.com/Home/Logon?urlkey=cffvrscholar). Recipients are required to provide supporting documents as instructed in the Scholarship Verification Form, such as class registration/list with credits listed, proof of major, grade transcript, etc. After approval of online Scholarship Verification Form, the scholarship check will be paid directly to the school the recipient will be attending. The Community Foundation communicates with students via email; it is the student’s responsibility to keep their Scholarship Portal account updated with their current email address.

Loss of Eligibility:

Failure to register as a student attending the UWO-Fox Cities campus, pursing a degree in Information Technology or Computer Science.
