Implementing Patient-centered Delivery of Clinical Preventive Services in Primary Care (PCOR TF)

This dissemination and implementation project will focus on increasing the equitable delivery of high priority clinical preventive services to adults through evidence-based, patient-centered models of primary care that: leverage team-based care, technology and informatics, and linkages with public health and community-based organizations; incorporate patient preferences and values; coordinate care; and reduce disparities.
