Overview

The proposed Collaboratory aims to promote national progress in the care of chronic disease, including among those living with multiple chronic conditions, by disseminating and implementing PCOR evidence to improve access, quality, and outcomes of care and address related health inequities. Funded health systems will commit to participate in a learning collaborative with other health systems and to implement one or more of a set of AHRQ proposed high-priority, evidence-based interventions that help improve care for chronic conditions within their affiliated primary care practices. The selected systems will also agree to work together to develop effective implementation strategies; facilitate learning and sharing of implementation best practices nationally; and test and evaluate implementation strategies.
